It seems that you have provided two different names based on your original request and this latest request. Can you confirm with us what is the name that is visible on your shop signboard?

Approved with some modifications. There is no need to input the full address. The fields are as it is, Street Name for the Street Name only and not including the unit number, which has its own field, and no Taman name should be included.
